This Consumer Health Information (“CHI”) Privacy Notice (“CHI Policy”) supplements Nurture, LLC’s (“Nurture,” “We,” “Us,” or “Our”) general privacy notice. This CHI Policy applies to personal data defined as CHI under applicable state privacy laws.
CHI We Collect
The CHI we collect depends on how you interact with us, the services you use, and the choices you make. Because CHI is defined very broadly, many of the categories of data we collect are or could be considered CHI.
Such data may also be categorized as follows:
Information about health conditions, diagnoses, symptoms, testing, medications, or treatment;
Information related to health insurance, reimbursement, benefits, or patient assistance programs;
Information about healthcare providers, prescriptions, or referrals;
Information that identifies an individual seeking products or services related to specific health conditions;
Internet or network activity, including interactions with websites or content, that could reasonably indicate interest in condition-specific products or health-related information;
Precise location information that could reasonably indicate an attempt to acquire or receive health-related products or services;
Communications that include health-related information provided by you;
Information collected in connection with patient support, adherence, or care programs;
Inferences derived from any of the above or from any other information to identify or predict health status or needs.
Sources of CHI
We may collect CHI from the following sources only as permitted by applicable law.
1. Directly from You
We may collect CHI directly from you and your authorized representatives when you:
Interact with our websites, mobile applications, or digital platforms;
Purchase or request information about products associated with specific health conditions;
Enroll in patient support, reimbursement, or assistance programs;
Communicate with us through customer service or other channels.
We may also collect certain information automatically through cookies, pixels, and similar technologies.
2. Healthcare, Reimbursement, and Support Partners
We may receive CHI from third parties such as:
Healthcare providers or care teams;
Pharmacies, specialty distributors, or fulfillment partners;
Health insurers, payors, or reimbursement support vendors;
Patient support or program administrators.
3. Service Providers and Processors
We may collect or process CHI through service providers that assist us in operating our business, including those that support:
Website and platform functionality;
Analytics and customer engagement tools;
Reimbursement, logistics, and customer service functions.
4. Publicly Available and Commercial Sources
We may obtain information from publicly available or commercially available sources and combine it with other information we collect to improve our products and services.
Why We Collect and Use CHI
We collect and use CHI to provide the products and services you request or authorize, or as otherwise permitted by applicable law. This may include:
Providing, fulfilling, and delivering products and related services;
Administering reimbursement, insurance, or patient support programs;
Communicating with you regarding your requests, orders, or program participation;
Operating, maintaining, securing, and troubleshooting our products, services, and systems;
Improving our products and services, including through internal analytics and research;
Providing information about products or services;
Personalizing communications or user experiences in a manner consistent with your expectations and applicable law; and
Complying with legal, regulatory, and reporting obligations;
Our Sharing of CHI
We may share each of the categories of data described above for the purposes described in our general privacy notice, to the extent permitted by law for CHI. For example, we may disclose your personal data, including CHI:
To the extent necessary to provide a product or service that you have requested or to complete any transaction you have requested or authorized, as described above.
For any purpose for which you consent or direct us to disclose it, for example marketing or advertising purposes.
Third Parties with Which We Share CHI
Depending on how you use our Services and the choices you make, we may share the categories of data described above with the following categories of recipients, as applicable and as permitted by law:
Processors. Vendors, service providers, or contractors (“processors”) that provide services on our behalf may access information that might be considered CHI for the purposes described above. For example, companies that provide customer service support, technical services, or assist in protecting and securing our systems and services may need access to data to provide those functions on our behalf.
Financial institutions & payment processors. When you obtain Services or enter into a financial transaction, we will disclose payment and transactional data to banks and other entities as necessary for payment processing, fraud prevention, credit risk reduction, analytics, or other related financial services.
Parties to a corporate transaction. We may disclose information that might be considered CHI as part of a potential or actual corporate transaction or proceeding such as a merger, financing, acquisition, bankruptcy, dissolution, or a transfer, divestiture, or sale of all or a portion of our business or assets.
Affiliates. We may share data or enable access to data across our subsidiaries, associated organizations affiliates, and related companies, to the extent access helps us to provide our services and operate our business.
Government agencies. As described in our privacy notice, we disclose data to law enforcement or other government agencies when we believe doing so is necessary to comply with applicable law or respond to valid legal process.
Professional advisors. We may disclose information that might be considered CHI to our professional advisors, such as lawyers, auditors, consultants, and other advisors, as reasonably necessary to obtain legal, compliance, audit, accounting, or other professional services.
Advertising, marketing, and analytics partners. Where permitted by law and, where required, with your consent or at your direction, we may disclose information that might be considered CHI to third parties that support our advertising, marketing, campaign measurement, audience insights, or analytics activities.
Other third parties. In certain circumstances, it may be necessary to provide data to other third parties, for example, to comply with the law or to protect our rights or those of our customers.
Other users and individuals. If you use our services to interact with other users of the Service or other recipients of communications, we may disclose data, including information that might be considered CHI, as directed and requested by you and your interactions.
The public. You may select and request options available through our Services to publicly display and disclose certain information, such as your profile, demographic data, content and files, or geolocation data, which may include information that might be considered CHI.
How to Exercise Your Rights and Set Your Data Preferences
In some states, CHI privacy laws provide certain rights to Consumers, including, but not limited to, rights to know, access, confirm the disclosure or sale, delete, or withdraw consent relating to such data, subject to certain limitations and exceptions. You may exercise these rights by submitting a request via our interactive webform or calling us at 1-866-866-0038. We will not discriminate or retaliate against you for exercising your rights under applicable privacy laws.
To opt-out of sales and processing of personal information for targeted advertising, you can also activate the Global Privacy Control (“GPC”) by implementing a browser or extension that offers this control. The GPC is a third-party tool. More information and instructions on implementing this tool are available here.
For requests submitted via telephone, you must provide us with sufficient information that allows us to verify you are the person about whom we collected the personal information and describe your request with sufficient detail to allow us to properly evaluate and respond to it. Our interactive webform is the most efficient way to submit a request and contains all information we require to process your request.
Depending on your state of residence, you may designate an authorized agent to make requests on your behalf by designating such an agent in writing or through a power of attorney. If you are an authorized agent making a request on behalf of another individual, you must provide us with signed documentation that you are authorized to act on behalf of that individual.

We may update this CHI Privacy Policy from time to time. When we do so, we will post the revised privacy policy on our website. Please check the revision date at the top of this page to determine if the policy has been modified since you last reviewed it.
